Replacing the cumbersome, old-fashioned paper document with a digital, interactive form. Analyzing the questions in the document, and splitting it into screens and navigating to the following screens, according to the user’s answers.

Instead of writing answers, interactive visual choice options.

Creating a step progress bar at the top of the screen, allowing the user to see which stage they’re in, and how far they’ve progressed. Data from the user’s previous answers is displayed on the right side in a designated area, as well as the option to go back and make changes when necessary. Finally, summary screen, terms and conditions screen and digital signature.
